Privacy policies

We comply with the Centers for Medicare and Medicaid Health Insurance Portability and Accountability Act (HIPAA) guidelines for patient confidentiality.

Disability claims

This office does not make determinations regarding disability. This determination requires specific training by people registered to perform such work. Any request for such a determination by this office will be denied.


Medication refills

24 hour notice is required for all medication refills. No medication refills of controlled substances are provided over the phone, or during night call coverage.


Appointments and cancellations

Same day appointments are made available for urgent cases that are referred from a physician. Otherwise, appointments are usually available within a 1-2 week period.
Cancellation of appointments should be communicated to our staff, preferably 48 hours prior to the time of the original appointment. This helps us to provide timely and efficient medical services.


Communications

Because of the unsecure nature of many email servers, we do not routinely use email at the present time. We are readily reachable by phone, and make every effort to communicate in a timely manner.


Referrals

It is your responsibility to assure that all necessary referrals have been requested from your primary care physician in advance. Without appropriate referrals (when and if needed), you will be responsible for any fees related to your care during that visit.


Payment

Copayments are required at the time of service. Cash, check, and credit (Visa, MasterCard, Discovery) are accepted. If you do not have health insurance coverage, we request payment for services on the day of provision.


Pre-certfication for diagnostic testing

Precertifications are now required for most diagnostic testing. If ordered by our physicians, our staff will strive to obtain such approvals. Studies requested by your referring physican, however, must be precertified by the ordering physician's office.
